    - General practitioners, specialists, allied health professionals, nurses, and school counsellors

    - Individuals looking to self-refer for support

    - Family members or caregivers assisting someone in their care

    - Managers, colleagues or workplaces

  • What happens after referral?

    Once this form is submitted, our team will review the information and reach out within 3–5 business days to discuss the next steps, including:

    Connection with QENDO’s free Nurse Navigator or Peer Support programs

    Access to educational tools, self-paced courses, and local peer groups

    Referrals to trusted clinical or mental health services (if applicable)
